Do you need a WPATH letter for top surgery in London?

It depends on which provider you use. WPATH letter requirements differ significantly between the NHS gender pathway, private providers that follow WPATH SOC strictly, and private providers that use a self-referral model.

Centre for Surgery — no WPATH letter required

For all gender-affirming surgery at Centre for Surgery — FTM top surgery, MTF top surgery, facial feminisation surgery, tracheal shave, body feminisation, body masculinisation, and non-binary surgery — adults aged 18+ self-refer directly. No WPATH letter, GP referral, gender clinic letter, or formal diagnosis of gender dysphoria is required.

A mandatory two-week cooling-off period applies after every consultation before surgery is booked.

NHS gender pathway — WPATH letter required

NHS gender-affirming surgery is accessed via the specialist Gender Identity Clinics (GICs). Patients on the NHS pathway are assessed over multiple appointments and receive WPATH-style assessment letters as part of the formal pre-surgical process. NHS waits are typically 12–18 months or longer for an initial GIC appointment, with additional time before surgery is offered.

Other private UK providers — variable

Some private providers in the UK require a WPATH letter; others do not. Check directly with the provider you are considering.

What is the WPATH Standards of Care?

The World Professional Association for Transgender Health (WPATH) is an international, multidisciplinary professional association that publishes the Standards of Care (SOC) for the health of transgender and gender-diverse people. The SOC is the most widely used international clinical framework for gender-affirming care.

WPATH SOC version 8 (current)

WPATH published Standards of Care version 8 in September 2022 — the current version. SOC v8 made several significant changes from previous versions:

  • Removed the “Real Life Experience” requirement — patients are no longer required to live in their affirmed gender role for 12 months before surgery
  • Removed mandatory hormone therapy duration requirements for top surgery
  • Streamlined to a single referral letter for top surgery (previously two were required for some procedures)
  • Emphasised informed consent models as appropriate for gender-affirming care
  • Recognised that referral letters need not come from mental health professionals in all cases — appropriately trained primary care or other clinicians may also provide them

If you are obtaining a WPATH letter, ensure your assessing clinician is working to SOC v8 — not the older SOC v7 framework which contains outdated requirements no longer recommended.

When might I want a WPATH letter even if not required?

Even though a WPATH letter is not required at Centre for Surgery, some patients choose to obtain one for other reasons:

  • Insurance reimbursement — some private health insurers require a WPATH-compliant assessment to consider reimbursement for gender-affirming procedures (most UK private insurers do not cover gender-affirming surgery regardless)
  • Future pathway flexibility — if you may later access NHS care or move to a country where WPATH letters are required, having one on file can be useful
  • Legal documentation — for gender recognition certificate applications or other legal matters
  • Personal preference — some patients value the additional assessment as part of their preparation for surgery

A WPATH assessment is not the same as the consultation you will have with your surgeon at Centre for Surgery. The surgical consultation focuses on assessing your anatomy, explaining the surgical technique, discussing realistic outcomes, and confirming whether the planned procedure is appropriate for you. The WPATH assessment is a separate clinical evaluation focused on confirming readiness for gender-affirming care more broadly.

How to obtain a WPATH letter in the UK

If you have decided you want or need a WPATH letter, the process is as follows:

1. Identify a qualified assessor

WPATH letters can be issued by any appropriately trained clinician working to SOC v8 — typically a psychologist, psychiatrist, GP with relevant training, or licensed mental health professional with experience in transgender healthcare. The assessor should be familiar with WPATH SOC v8 specifically.

UK options include:

  • NHS Gender Identity Clinics — the GIC pathway provides comprehensive assessment but with significant waits (12–18+ months for initial appointment)
  • Private gender identity specialists — independent psychologists, psychiatrists, and GPs with transgender healthcare experience offer WPATH-compliant assessment privately, typically within weeks. Costs vary; expect £200–£500 for a single-letter assessment
  • UK organisations supporting transgender healthcareGendered Intelligence, London Friend, and CliniQ provide community resources and can signpost to qualified assessors
  • Professional directories — the British Association for Counselling and Psychotherapy (BACP) and UK Council for Psychotherapy (UKCP) maintain searchable directories of registered practitioners

2. Schedule the assessment

The assessment is typically a single appointment of 60–90 minutes, sometimes split across two appointments depending on the assessor. The clinician will discuss your gender identity, history, current circumstances, the procedure you are considering, your understanding of risks and benefits, and your readiness to proceed.

3. Receive your letter

If the assessor concludes you meet WPATH SOC v8 criteria, they will issue the letter — typically within 1–2 weeks of the assessment.

What is in a WPATH-compliant assessment letter?

A WPATH SOC v8 letter for top surgery typically includes:

  • The clinician’s name, qualifications, and registration details
  • The date of assessment and date of letter
  • Confirmation that you meet WPATH SOC v8 eligibility criteria for the specific procedure
  • Confirmation that any significant medical or mental health concerns are reasonably well-controlled
  • Confirmation that you have capacity to provide informed consent
  • Confirmation that you understand the risks, benefits, alternatives, and limitations of the planned procedure

The letter does not require photographs, financial documentation, or proof of identity — these are not WPATH requirements. Be cautious of any assessor who demands documents not specified in SOC v8.

Is the “Real Life Experience” still required for a WPATH letter?

No. WPATH SOC v8 (September 2022) removed the Real Life Experience (RLE) requirement. Some older guidance documents and outdated assessors may still reference it, but it is no longer part of current WPATH standards. If an assessor insists on RLE, they are not following current SOC v8.

Do I need to be on hormone therapy to get a WPATH letter for top surgery?

No. WPATH SOC v8 does not require a specific duration of hormone therapy before top surgery, and hormone therapy is not a precondition. Many patients have top surgery without ever taking hormones.

What if my WPATH assessment outcome is not supportive?

If an assessor concludes you do not currently meet criteria, they should explain why and outline what additional support, time, or steps might be appropriate. You can also seek a second opinion from a different qualified assessor.

Does Centre for Surgery accept WPATH letters from other clinicians?

We do not require a WPATH letter, but if you have one we are happy to add it to your medical record alongside the standard CFS pre-operative assessment. The WPATH letter does not replace or shorten our own pre-operative assessment process — both your surgical consultation and the mandatory two-week cooling-off period still apply.

Top surgery at Centre for Surgery — what is required

To proceed with top surgery at Centre for Surgery you need:

  • To be aged 18 or over
  • To attend a face-to-face consultation with one of our gender-affirming chest surgeons (Dr Spiros Vlachos or Mr Andreas Shiatis)
  • To complete the mandatory two-week cooling-off period between consent and surgery
  • To be in good general health, with no uncontrolled conditions affecting anaesthetic risk
  • To stop smoking for at least four weeks before and after surgery
  • To have realistic expectations of what surgery can achieve

You do not need:

  • A WPATH letter
  • A GP referral
  • A gender clinic referral
  • A formal diagnosis of gender dysphoria
  • A psychological assessment
  • To be on hormone therapy
  • A specified period of “lived experience” in your affirmed gender
